New Patient Registrationregister_now_blue_sign

If you are new to the area and wish to register at Loughview Medical Centre, please call in for a registration pack or download the registration forms.  Please send (or drop in) the forms together with proof of ID (passport or driving license) and proof of address.

You will also be asked to complete a medical questionnaire when registering as this allows the doctor to familiarise him/herself with your medical history and any current medication.

All new registrations are then reviewed by our GP partners.  Following approval, you will be invited to attend a registration appointment which will include a brief medical check.

If you change your name, address or telephone number, please let us know and we will then update our records.  When changing address please check with the receptionist that you still live within our catchment area.

If you have difficulty getting registered in the area you may be assigned to a GP by contacting Business Services Organisation 2 Franklin Street, Belfast, BT2 8DQ. Telephone no 03005550113.

Practice Boundaries

The practice area extends to approximately six miles from Kircubbin.

Policy for Accepting and Removing Patients

This practice has an open policy for accepting patients within our catchment area without prejudice to age, sex, health, nationality or race.  Patients are removed from any doctor’s list, only if there is an episode of violence, unsocial behaviour in our premises or during a home visit, a breakdown in doctor/patient relationship, persistent failure to attend appointments or patients move out of our practice area.

Guide to GP Services
